GreenLeaf Care Limited provides a responsive, professional referral service to local authorities, social workers and partner agencies. We aim to match children and young people to the right home quickly while ensuring safety, stability and the best possible outcomes.

Referral pathway

Initial enquiry — contact our Referrals Team to check availability and suitability.

Referral submission — complete the referral form and provide relevant documents (social history, reports, risk assessments).

Multi-disciplinary review — our team reviews clinical, educational and behavioural needs to determine match.

Planning meeting — for planned placements we arrange a planning meeting or pre-placement visit.

Placement commencement — care plan and PBSP are finalised with the placing authority.

Ongoing review & communication — regular updates and statutory reviews in partnership with the placing authority.

Emergency placements

We operate a clear process for urgent referrals and will advise on availability. Emergency placements are considered on a case-by-case basis subject to matching and risk criteria.
